𝐩𝐑𝐁𝐂𝐬 ⁣
🔴 Transfusing 1 unit of pRBCs raises Hgb ~ 1g/dL & Hct by 2-3%. This provides a good estimate of where you should be at without drawing labs.⁣
⁣
🔴 There are 𝘢𝘥𝘥𝘪𝘵𝘪𝘷𝘦𝘴 that increase the shelf-life of blood. ⁣
- 𝐂𝐢𝐭𝐫𝐚𝐭𝐞: anticoagulant. You don’t want your blood clotting in the bag! It inhibits calcium. Keep this in mind if giving multiple units, could lead to hypocalcemia. ⁣
- 𝐃𝐞𝐱𝐭𝐫𝐨𝐬𝐞: substrate for glycolysis for energy (ATP). This prevents the RBC from using the oxygen they’re transporting to the tissues. ⁣
- 𝐏𝐡𝐨𝐬𝐩𝐡𝐚𝐭𝐞: a buffer that prevents acidosis⁣
⁣
🔴 Although these additives are great to prolong banked blood, they also come with consequences the longer the blood sits there. ⁣
⁣
𝐑𝐢𝐬𝐤𝐬 include:⁣
- ↓ 𝐩𝐇 = ↑ lactic acid = bad⁣
- ↑ 𝐊 (keep in mind for renal fail pts). Hyperkalemia can result from an impaired RBC cell membrane that happens with stored blood, causing K to leak out. ⁣
- ↓ 𝟐,𝟑 𝐃𝐏𝐆 = ↓O2 release to the tissues. Which is the whole point of RBCs right. To deliver oxygen to the body. This is why with ECMO patients we used to ask blood bank for our blood to be <10 days old, to get the max benefit for these sick hypoxic patients. ⁣
⁣
🔴What does 𝘭𝘦𝘶𝘬𝘰𝘳𝘦𝘥𝘶𝘤𝘵𝘪𝘰𝘯 mean? You’ll see this a lot on your blood bags. Leuko = WBC, reduced. This reduces/removes WBCs from pRBCs. This decreases the risk of febrile transfusion reactions & other adverse effects. ⁣

It was a cool day. I remember the first time an an It was a cool day. I remember the first time an anesthesiologist went to intubate and he let me peek at the vocal cords when I was 15. I fell in love with anesthesia that day. 

Early on in clinical, I would think of that moment sometimes & wonder how he was able to do that so gracefully. I remember being so overwhelmed with intubating - how was he able to keep that perfect view for so long and so casually? I felt like I had seconds before I would lose my view and struggle to find it again. I felt like that would never be me and that I was a failure. 

I remember being asked by preceptors to explain each step of what I was doing during induction, and how my mind would literally black out cause I was trying so hard to focus on the tasks. How could they expect me to say everything out loud and the rationale plus answer their questions during such a critical moment? 

Today there were 2 nursing students shadowing in the OR, & my preceptor told me to explain everything I was doing & then I did. And when I went to intubate I got my view & asked them both if they wanted to see & then they did. And idk if I’m emotional because I’ve had 3 hours of sleep or cause that was just kinda cool because I finally felt competent enough to do what that MD did 15 years ago but yeah it was a good day.

Must Haves: Mascara

I thought I’d start off my beauty section of the blog with one of my favorite makeup products: mascara! Mascara is something most girls don’t leave the house without and with so many options out there, it really is difficult to find the right ones. Now, I believe that some mascaras look amazing on some and not so much on others. Everyone’s lashes are different. But, these are the mascaras I find look the best on myself, and majority of people I put them on.

In no particular order…

1. “They’re Real” by Benefit Cosmetics – Alriiiight I’ll start off with my bias but since working for Benefit years ago this mascara really is incredible. I mean, I have put this mascara on thousands of people and not once did it disappoint. It gives volume, length, and curl, even on straight lashes. It has a plastic wand with bristles on the side but also vertically on top of the wand. This way, after applying, push the lashes up vertically for even more lift. It is “water-resistant.” They’re Real comes in black, brown, and blue, and has a waterproof version as well (even though the original is pretty dang waterproof anyways). The one flaw about this mascara is that while it does not smudge at all, it is difficult to remove. You’ll need a makeup remover that works for waterproof makeup or you can buy Benefit’s “They’re Real remover,” a cream. People have asked me numerous times if my lashes were real so it lives up to its name. Cost = $24 for 0.3oz. Buy at Sephora Here.

2. “In Extreme Dimension – 3D Black Lash” by MAC – This mascara was released shortly after “They’re Real” came out and you can tell they modeled it after just that. But, it comes pretty close to “They’re Real” and is not as difficult to take off, thus it’s not water-resistant. The wand is very similar, with plastic bristles horizontally and vertically, to capture every lash. This one is great for length and curl, with slightly less volume than “They’re Real”. But it has this MAC smell I just love. Comes in black, and there is a waterproof version as well. Cost = $23 for 0.44oz. 3. “Better than Sex” by Too Faced – While opinions may vary on the name, this mascara has become a favorite of many since it came out. The hourglass shaped brush creates lots of volume, giving you a doll eyed look. I’ve heard from some that this mascara is too thick for them and appears clumpy. While I agree a little, if you mess with your lashes well enough clumps can separate into thick black lashes. There’s also a waterproof version I have as well. Paraben free. Cost = $23 for 0.27oz. Buy at Sephora Here.

4. “Hypnose Drama” by Lancome – I love this mascara. This mascara is great for length and volume, and with its s-shaped brush, it captures every lash. This mascara gives you thick spidery looking lashes. “Hypnose Drama” comes in just black, and there’s a waterproof version. Cost = $27 .50/0.22oz. Buy at Sephora Here.

    This is a great list of mascara! I’m almost out, so I will need to try one of these! Currently, my favorite mascara is L’oreal Telescopic because it makes my lashes longer and holds pretty well!

    My favorite mascara is Lash Love Mascara by Mary Kay. Plus I wear false lashes, and it makes them look really long and luxurious!

    I have never worn a mascara. To me it was a waste of resources to have one in my kit. After reading this post. I fell like I’ve never really been fair to my self. Thank You
